Adult Membership

For adults 27 y/o and over

INVESTMENT: $3,600 /YEAR

    • Annual wellness physical examination, labs included

    • Comprehensive review of your medical history

    • Management of chronic conditions

    • Women’s Health Care including breast exams and pap smears

    • Electrocardiogram (ECGs) Testing

    • Anxiety and Depression Screening

    • Cognitive Screening

    • Convenience of in-office lab draws (billed directly to your insurance by the Northwestern Health lab)

    • Same day or next day appointments either in-person or virtual (when feasible and advisable) via our secure, HIPAA-compliant platform

    • Point of Care (POC) testing completed same day in our office with immediately available results such as rapid strep, rapid flu, rapid covid, glucose, mono, pregnancy and urinalysis

    • Timely communication to address questions or concerns between visits via our secure HIPAA-compliant platform and/or patient portal

    • Coordination of medical care with your specialists

    • Weight Management

    • Sports and Work Physicals

    • Pre-Operative Evaluations

    • Coordination of medical care with your hospital team

Pediatric Membership

For children and young adults 26 y/o and under

INVESTMENT: $3,600 /YEAR

    • Unlimited, regular, age-appropriate checkups to monitor growth, development, and milestones.

    • Same day or next day appointments for common childhood illnesses such as colds, ear infections, or strep throat.

    • Regular assessments of cognitive, physical, and emotional milestones

    • Guidance on establishing healthy habits for kids (exercise, nutrition, screen time).

    • Age-appropriate medication prescriptions and management.

    • Special attention to vaccine schedules and catch-up plans for children who may have missed doses.

    • Screening for conditions like scoliosis, lead poisoning, and anemia.

    • Pediatric-focused sports physicals

    • Point of Care (POC) testing completed same day in our office with immediately available results such as rapid strep, rapid flu, rapid covid, glucose, mono, and urinalysis

    • Timely communication to address questions or concerns between visits via our secure HIPAA-compliant platform and/or patient portal

    • Coordination of medical care with your specialists

Weight Management

This excludes all other primary care services

INVESTMENT: $150 /MONTH

    • This is a great option for those that are not ready for full membership but just feel they want to work on weight management.

    • Get monthly consults with Dr. Marabella either in person or virtual

    • Complete medical history and physical with labs, reviewing medications, and family history

    • Physician supervised weight loss

    • Injectables (semaglutide/tirzepatide) as well as oral medications. Member only pricing available.

    • Referral to dietician and personal trainer

    *Excludes all other primary care services

  • Yes, currently Blossom Family Medicine is affiliated with Endeavor Hospital (formally NCH) & Advocate Good Shepherd

  • If an emergency happens, Dr. Marabella is available for consultation with any emergency care physician. Emergency room care will be billed to your insurance.

  • Dr. Marabella is on the Medical Staff at Endeavor hospital and has access to your medical records from the hospital, however the hospital physician will be the care provider while you are in-patient. They will communicate with us while you are hospitalized. If you are hospitalized at a different hospital Dr. Marabella will communicate with their hospitalist and get your records as well.

  • Blossom Family Medicine does not accept insurance payment for our medical services. Rather, our stated services are covered by the annual fee.

  • Yes! Your insurance will be billed directly for lab work, any diagnostic testing not done in our office, specialist visits, emergency room visits and hospitalizations.
