An initial visit to your doctor to complete a comprehensive review of your medical history and to perform a physical examination. During this visit you and your doctor will collaborate on developing your plan of care.
An annual physical examination with associated lab tests conveniently drawn on-site.
Women’s health care including breast exams and pap smears
Sick-visits as needed
Follow-up visits as needed
Management of chronic conditions and associated in-person or virtual visits
Coordination of medical care with your specialists
Convenient on-site Point of Care (POC) testing with immediately available results including urine dipstick, pregnancy testing, mono, strep, covid and flu testing
Convenient on-site EKG as part of the annual physical examination when indicated

Our price model is simple and straightforward, with one annual fee that covers all the services we provide and a one-time registration fee. There are no additional copays or fees. If you are interested in becoming a patient, please contact our office to begin your journey.

At Blossom Family Medicine, our annual membership is thoughtfully designed to deliver high-quality, comprehensive healthcare that puts you first:
Single Adult Membership: $3,600/year
Family Membership: $3,600/year per adult, with a discounted rate of $2,500/year for each child or young adult aged 6–26.
A one-time registration fee of $250 per patient applies. You can choose to pay the full amount upfront or spread the cost over monthly installments—whichever works best for you.

Yes, currently Blossom Family Medicine is affiliated with Endeavor Hospital (formally NCH)

If an emergency happens, Dr. Marabella is available for consultation with any emergency care physician. Emergency room care will be billed to your insurance.

Dr. Marabella is on the Medical Staff at Endeavor hospital and has access to your medical records from the hospital, however the hospital physician will be the care provider while you are in-patient. They will communicate with us while you are hospitalized. If you are hospitalized at a different hospital Dr. Marabella will communicate with their hospitalist and get your records as well.

No, your doctor will be accessible and appointments will be readily available. She will have a limited number of patients in her panel in order to ensure that we can provide specialized care to each and every patient.

We accept patients 6 years and older.

Blossom does not accept insurance payment for our medical services. Rather, our stated services are covered by the annual membership fee.

Yes! Your insurance will be billed directly for lab work, any diagnostic testing not done in our office, specialist visits, emergency room visits and hospitalizations.
